Оправдано ли назначение антидепрессантов при биполярной депрессии с точки зрения доказательной медицины?

Обоснование исследования: в настоящее время вопрос назначения антидепрессантов при лечении биполярной депрессии остается весьма спорным в связи с противоречием между рутинной клинической практикой и результатами контролируемых исследований. Цель исследования: оценка обоснованности назначения антидепрессантов при биполярной депрессии с точки зрения доказательной медицины. Материал и методы: проведен поиск по базам данных (Scopus и MEDLINE) по ключевым словам «bipolar depression and antidepressants». В анализ включались исследования эффективности антидепрессантов при биполярной депрессии (в том числе из списков литературы метаанализа по данной теме с 1999 г.), в которых участвовало как минимум 30 человек. Из 734 результатов поиска было отобрано 23 исследования (из них 13 плацебо-контролируемых и 10 сравнительных). Заключение: отмечена высокая степень противоречивости результатов, по-видимому, в значительной степени связанная с методологией. Всего два исследования сравнивали эффективность антидепрессантов в монотерапии с плацебо, при этом в исследовании с 740 участниками отличий найдено не было, в исследовании с 70 участниками с биполярным расстройством 2 типа антидепрессанты оказались эффективнее плацебо. Тем не менее в обоих исследованиях были выявлены достаточно весомые методологические проблемы. В 6 исследованиях, сравнивавших эффективность сочетания антидепрессантов с нормотимиками против сочетания нормотимиков с плацебо, была выявлена только эффективность флуоксетина в сочетании с оланзапином, другие антидепрессанты оказались неэффективны. В то же время в исследованиях, где антидепрессанты сравнивались друг с другом в комбинации с нормотимиками, был выявлен значимый клинический ответ на терапию. Анализ исследований рутинной клинической практики показал, что антидепрессанты назначаются в среднем 50% пациентов с биполярной депрессией. Международные рекомендации по терапии достаточно сильно расходятся в вопросе применения антидепрессантов при биполярной депрессии, но в принципе допускают их назначение. Таким образом, несмотря на противоречивость литературных данных, назначение антидепрессантов при биполярной депрессии является оправданным с точки зрения доказательной медицины для определенных групп пациентов при учете факторов риска.
